End-of-life planning

Advance directive planner

Recording your wishes now means that if you are ever unable to speak for yourself, your care team and loved ones will know exactly what you want. This is one of the most important gifts you can give your family.

Why plan ahead?

Peace of mind for your family

When families know your wishes, they are spared the burden of guessing or disagreeing under enormous emotional pressure. Your clarity is a gift of love.

Your wishes are honoured

Documented preferences give healthcare professionals clear guidance and legal authority to follow your wishes, even in an emergency or if you lose capacity.

You can change your mind

An advance directive is not permanent. You can update, amend, or revoke your preferences at any time, as long as you have capacity to do so.

Demonstration planner

This is a demo site — no real legal document is created and no data is submitted to a healthcare system. Completing this form writes a sample entry to your browser's localStorage. In a live system, completed preferences would be stored securely, shared with the care team, and attached to the patient record.

Personal details

Basic information to identify this document. In a real system, this would be linked to your patient record.

Healthcare proxy

A healthcare proxy (also called a healthcare agent or lasting power of attorney) is someone you appoint to make medical decisions on your behalf if you are unable to do so.

Resuscitation (CPR)

Cardiopulmonary resuscitation (CPR) is used when the heart or breathing stops. In a hospice context, many patients choose a do-not-resuscitate (DNR) preference to allow a natural death.

Artificial life support

Life support includes mechanical ventilation (breathing machines), artificial nutrition and hydration, and other interventions to keep the body functioning.

Preferred care setting

Where would you prefer to be cared for, and where would you prefer to die, if it is at all possible?

Comfort & values

Help us understand what matters most to you at the end of life so that our care team can tailor every visit.

Organ and tissue donation

Your wishes about organ and tissue donation, to be shared with your care team and family.

Additional wishes

Any other preferences, notes for your care team, or instructions you would like to record.

Record when you completed this document. Any amendments should be dated.

Preferences saved

Your advance directive preferences have been recorded. Our social worker will contact you to discuss next steps and ensure these are properly documented in your care plan.

Demonstration acknowledgement This is a sample website built by RG Consulting to demonstrate how a hospice advance care planning system could work for your organisation. No real legal document has been created. The sample preferences have been written to your browser's localStorage — visit the admin dashboard to see how advance directive submissions appear in the care management view.
